Aging and Disability Resource Connection (ADRC)

Short-Term Service Coordination

  • Short-Term Service Coordination is personalized service coordination for the purpose of stabilizing a situation for individuals whose health, safety and welfare are at risk.
  • Assistance to prevent unnecessary admittance to emergency department or institutional placement.
  • Usually lasts 90 days or less.
  • After stabilization of consumer’s emergency, referral to options counseling for LTSS planning is available.

  • Empower individuals and families to make informed decisions.
  • Streamline access to a wide variety of services.
  • Serve individuals, professionals and the community as a visible, trusted resource for information help.

The ADRC saves time and reduces frustration by providing a single point of contact. Working with our ADRC staff will help to streamline your efforts to get the information and services that you are looking for. The ADRC is especially helpful in assisting someone who has multiple needs and may need help from multiple resources. Our goal is to connect you as quickly and easily as possible with what you need. The ADRC is the best first option for information on aging and disability related needs in Orange County.

Transportation

Putting the brakes on elderly driving.

Senior Mobility Program. Westminster On Wheels (WOW) Monday through Thursday offers the Nutrition Van Program, curb-to-curb transportation to and from the Westminster Senior Center (which offers a lunch program), and then the WOW Shuttle to the following shopping locations: Albertson’s, Stater Brothers, Wal-Mart, Westminster Center, Westminster Mall, 99 Cent Store and Target.

Paratransit: OCTA ACCESS eligibility office at 714-560-5956. “OC ACCESS is a shared-ride service that is available to qualified applicants whose physical or cognitive limitations prevent them from utilizing the regular OC Bus fixed-route service.”

Food

Meals on Wheels, an organization located in Anaheim, provides meals to the home bound seniors in Westminster. For more information or to apply for the program, contact Meals on Wheels at 714-220-0224.
