Death Certificate

In Orange County

• The attending physician is responsible for signing the death certificate. This is the physician who was in charge of the patient’s medical care for the illness or condition that led to the death.
• Attending Physician: The physician must be the attending physician who last cared for the patient.
• Medical Knowledge: The physician should have sufficient knowledge of the patient’s medical history to accurately determine the cause of death.

PEG

• Get 2026, 2024, and any previous records from Hoag, Newport Beach. Ask to keep MLF’s file active.
• Get 2026, 2020, and any previous records from MemorialCare, Fountain Valley. Ask to keep MLF’s file active.
• Ask Dr Bhakta about pulmonary-related imaging and treatment dates and locations. Ask to keep MLF’s file active. Per phone conversation with PEG, MLF never shared his naval service or consequent asbestos exposure. Dr. Bhakta will be the point of contact for completing the physician portion of the death certificate (rather than Kindred). Dr. Bhakta will review his files for his notes from Dr. Singh since Dr. Singh has been MLF’s doctor for pulmonary care.
• Get records from OC Urgent Care.
• Ask Charles An about:
  • Purchasing the casket for MLF from Overnight Caskets and about putting one on layaway for YSF.
  • Calling the VA Cemetery Scheduling 800 number.
  • Contacting Dr. Bhakta and/or Kindred regarding the cause of death (COPD caused by pulmonary asbestosis).
  • Contacting SSA to discontinue monthly payments.
  • Contacting DFAS to discontinue monthly payments to MLF (and to start survivor payments to YSF).
• Start the process to get MLF’s VA medical records.
• Determine if SSA has been paying more to YSF or to MLF.
• Determine if MLF was paying for YSF to get half of MLF’s retirement pay.
• Determine what pension payments need to be changed.
• Schedule bill payment.
• Paperless and auto-pay. Electronic access. Add PEG cell and email.
• Check with Fran on continuing to use MLF’s iPhone for online verifications.

RMF

• Take keys from dining room to Wells Fargo, 6952 Bolsa Ave, HB, and determine if any are the missing safe deposit box key/s. If yes, then open the safe deposit box, retrieve the grey accordion folder, and photograph :
  • Under ‘M’: Mel’s military and military retirement records, Mel’s birth certificate
  • Under ‘P’: GA and CA property deeds
  • Under ‘Y’: Yoshi’s naturalization certificate
  • Envelopes: marriage certificate, financial POA, revocable living trust, Mel’s will, Mel’s SSA card, Mel’s birth certificate
• If no key match, then photograph a sample key, and ask about opening the box without a key. At Barney, the other place to look is in the china dishes (in the covered gravy boat???) toward the front of the bottom or middle shelf of the glass-fronted china cabinet.
• Go to Navy Federal Credit Union. Close MLF’s VISA (with YSF as an authorized user) and open a new VISA with YSF as the account holder (and RMF as an authorized user???). Open a new savings (checking?) account linked to the trust. ATM card for YSF (and for RMF?).
• Introduce Fran to Mayu and Ikuko.
• Introduce Fran to Charles An.
• Introduce Fran to Beachside staff.
• Take Yoshi and Fran to see Baby at Amigo Animal Hospital.
