2026/7/22

• SoCal Gas – called 1-800-427-1167 M – F, 7am to 3pm PT to change account owner name to trust. Uploaded death certificate and trust to SoCal Gas portal.
• Equifax – wrote letter with required information to report death of MLF.
• Called Super Care Health to arrange for return of oxygen concentrator and tank.

2026/7/20 – 21

• State Farm CA – spoke with Kim Jones to update contact information and payment for policies for Barney, Barney earthquake (CEA), YSF drivers, Dodge Caravan, Nissan van. Followed up with email to Kim Jones with trust name and origination date.
• State Farm GA – spoke with Shawn at Lisa Craddock’s office to update contact information and payment for policy for GA rental property and to add umbrella. Will need to follow up with Shawn / Lisa after GA trust and probate are complete.
• SoCalGas – called to change account ownership to YSF name, MLF iPhone, YSF gmail. Start of service 2026/7/17, no charge for the change. Applied for the care assistance program on the basis of Medicare enrollment. Setup paperless and autopay.
• SoCalEdison – called to try to change account ownership to YSF name, MLF iPhone, and YSF gmail. Followed up with email to the SCE Deceased Department with death certificate and the new account ownership information. Created login with YSF gmail.

2026/5/26

PEG

• Get 2026, 2024, and any previous records from Hoag, Newport Beach. Ask to keep MLF’s file active.
• Get 2026, 2020, and any previous records from MemorialCare, Fountain Valley. Ask to keep MLF’s file active.
• Ask Dr Bhakta about pulmonary-related imaging and treatment dates and locations. Ask to keep MLF’s file active. Per phone conversation with PEG, MLF never shared his naval service or consequent asbestos exposure. Dr. Bhakta will be the point of contact for completing the physician portion of the death certificate (rather than Kindred). Dr. Bhakta will review his files for his notes from Dr. Singh since Dr. Singh has been MLF’s doctor for pulmonary care.
• Get records from OC Urgent Care.
• Determine what pension payments need to be changed.
• Schedule bill payment.
• Paperless and auto-pay. Electronic access. Add PEG cell and email.

RMF

• Wells Fargo, 6952 Bolsa Ave, HB, safe deposit box, retrieve the grey accordion folder, and photograph:
